Fall protection is the most-cited category in OSHA's annual Top 10 violations list, most years running — and not because the rule is unclear. It's because "fall protection" isn't one requirement, it's a set of thresholds and exceptions that shift depending on the work being done, and crews often apply the wrong threshold for their trade.
The Baseline: 6 Feet in Construction
Under 29 CFR 1926.501, general construction work requires fall protection — guardrails, safety nets, or personal fall arrest — at 6 feet above a lower level. This is the number most people know. It's also the number that gets misapplied, because several trades have their own specific thresholds that override it.
Where the Threshold Changes
- Steel erection (Subpart R): the general threshold is 15 feet, not 6 — but connectors have a specific, limited exception under 1926.760(b) allowing brief work without conventional fall arrest under defined criteria. This exception is narrower than most crews assume, and is often over-applied to work it doesn't cover.
- Scaffolds (Subpart L): guardrails or fall arrest are generally required above 10 feet, with additional requirements for suspended scaffolds regardless of height.
- Residential construction: the 6-foot threshold applies, but OSHA's residential construction guidance (1926.501(b)(13)) allows alternative methods when conventional fall protection is infeasible — with specific written justification required, not just a judgment call on site.
- Roofing work: low-slope and steep-slope roofs have different acceptable systems — warning line systems are permitted on low-slope roofs at least 6 feet from the edge, but steep-slope roofing generally requires guardrails, safety nets, or personal fall arrest with no warning-line alternative.
The Part Everyone Skips: Rescue Planning
A personal fall arrest system that stops a fall isn't the end of the hazard — suspension trauma can become life-threatening within minutes of a worker hanging in a harness. OSHA requires a prompt rescue plan wherever fall arrest is in use, and "call 911" is not, by itself, an adequate plan if response time is longer than the window before suspension trauma sets in. If your fall protection plan doesn't name who performs rescue and how, it's incomplete regardless of how good the anchor points are.
Anchor Points: Rated, Not Assumed
A common site failure is treating any convenient structural member as an anchor point. OSHA requires anchors used for fall arrest to support 5,000 lbs per attached worker, or be part of a certified engineered system with a safety factor of at least two. "It looks strong enough" is not a rating.
Getting the Threshold Right for Your Trade
The single most common fall protection mistake isn't skipping protection — it's applying the wrong trade's threshold. A carpentry crew treating 6 feet as universal will over-protect some tasks and, more dangerously, under-protect ones governed by a different subpart entirely. Know which standard actually governs the work in front of you before deciding what protection applies.
